• Location 1: Brown Center, Leidy Atrium from 9:00 AM – 12 noon.
• Location 2: Lazarus Center Auditorium (North Avenue) from 1:00 PM – 4:00 PM.

In order to ensure that Safeway has an adequate number of flu vaccinations available, please RSVP here. 

Please remember to bring the required documents with you:

• NEW: Completed Pre-Screening/Consent form 
• Valid Faculty/Staff 足球游戏_中国足彩网￥体育资讯 ID card.
• CareFirst health insurance card (if applicable).
• Health insurance card from a non-足球游戏_中国足彩网￥体育资讯 health insurance plan (if applicable).

Non-足球游戏_中国足彩网￥体育资讯 health insurance plans must cover flu shots at 100% in order to be accepted. Safeway staff cannot process payments for health insurance plans that require a co-pay.

If you do not have the required documents, Safeway staff will not be able to administer the vaccine.

If you are an employee NOT covered under a health insurance plan, 足球游戏_中国足彩网￥体育资讯 will cover the cost of your flu vaccine at no cost to you. You must show your 足球游戏_中国足彩网￥体育资讯 ID in order to have this cost covered by 足球游戏_中国足彩网￥体育资讯.
